1 - AUTHORISATION
The authorisation, necessarily signed by both parents (except in other compliant cases ordered by the Judicial Authority), is the document that authorises the girls/boys to participate in our activities, and protects the leaders in the conduct of the same.
2 - ID BOTH PARENTS
Each parent (except in other compliant cases ordered by the Judicial Authority) provides his/her parent with a front/back photocopy of their identification documents.
For citizens of the European Union, the European Economic Area, the Swiss Confederation and the Republic of San Marino
Valid identity card or we accept one of the following valid documents.
For citizens of a non-EU/EEA/CH/RSM country
we accept one of the following documents
- EU/EEA/CH/RSM identity card
- Passport with visa valid for stay in the Schengen Area
- Diplomatic passport and Vienna Convention *
- Geneva Convention status document **
- Residence permit **
- Residence card **
- EU Blue Card **
Any document must be valid.
Your daughter’s identification document is NOT required
* Only diplomatic passports of nations/institutions/territories legally recognised by the Ministry of Foreign Affairs and International Cooperation of the Italian Republic are accepted.
** Documents marked with an asterisk must necessarily be issued by an EU/EEA/CH institution.
3 - MEDICAL DATA SHEET
In the first part of the medical form, indicate your daughter’s health data (e.g. any allergies or intolerances, last surgical operations, …), while in the second part, fill in the consent to the processing of her health data.
4 - HEALTH INSURANCE
If your daughter or son is covered by European Health Insurance, please make a double-sided photocopy of his or her health insurance card.
We only accept the following health insurances
- EU/EEA/CH/UK residents: one of these documents
- Residents of San Marino: ISS registration number
- Residents in other countries/territories: we only consider as valid health insurance policies of a third country that has an agreement with the National Health Service and is able to meet the expected treatment and assistance requirements. More information at this link.
We do NOT accept any other health cover if this does not cover the cost of access to the National Health Service of the Republic of Italy.
5 - VACCINATION BOOKLET
Bring a photocopy of your child’s vaccination booklet.
If your child is insured through the Health Service of the Region of Tuscany, you can also download it here.
WARNINGS:
- We only accept vaccinations carried out in an EU/EEA/CH/UK country or otherwise recognised by the National Health Service
- In order to participate in our activities it is mandatory to be in good standing with a tetanus vaccination (including booster)
6 - PRIVACY POLICY
Information on the processing of your daughter’s personal data by our association in accordance with the GDPR.

If your daughter is already registered in our association and in the same unit (e.g. in the same cubs, …) this year, please follow these instructions:
